The program will provide approximately $15 million per year over three years, with individual grants ranging from $75,000 to $300,000 annually. Funding is based on the number of case manager positions requested, with a maximum of $150,000 per full-time equivalent (FTE) position. Applicants can request at least 0.5 FTE positions but no more than the number approved in their current grant. Demonstrating the need for the requested funding is required.
Applications are due by **4:00 p.m. Eastern Time on February 26, 2025**, with no exceptions for late submissions. To apply, organizations must have an EIN, a UEI from SAM, and active SAM registration. Applications must be submitted electronically through the GPD website—other submission methods will not be accepted.
